This role requires a collaborative approach to anticipate, integrate, and coordinate patient care plans, maximizing the ability to remain in a community setting.Key Responsibilities

• Identify at-risk patient populations who require care management services, in collaboration with physicians.
• Serve as a patient/family advocate, acting as a communication link between the physician, patient, family, and interdisciplinary healthcare team, as well as community resources.
• Support the mission, values, and management of patient care services, actively incorporating ANA Scope and Standards of Nursing Practice, North Carolina Board of Nursing Practice Act, WakeMed's Division of Nursing Professional Practice model, and WakeMed's Patient & Family Centered Care (PFCC) model.Requirements
  • 8 years of experience in a nursing-related area.
  • Bachelor's Degree in Nursing required.
  • Registered Nurse licensure required.Work ScheduleMonday-Friday, with variable hours. No weekends or call requirements.
