Job Description:
Description Provide high-quality nursing care to residents
following established facility policies and procedures. Assist in
the care and management of nursing units. Regularly check assigned
unit to ensure residents receive prescribed care. Make
observations, maintain charts, and order supplies as necessary. All
duties and responsibilities performed within the scope of Nursing
Practice for Licensed Practical Nurses in North Carolina. Duties:
Receive and provide resident status report during shift changes.
Conduct routine checks on residents, noting any changes in
condition and reporting to RN Supervision. Notify attending
physician and family of resident illnesses or injuries. Complete
accident and incident reports; submit to Director of Nursing.
Maintain positive interactions with residents, visitors, family
members, and staff. Assist in admitting, discharging, and
transferring residents under Nursing Supervisor guidance.
Contribute to the development, implementation, and review of
resident care plans. Take verbal or telephone orders from
physicians; transcribe to physician order sheets in the absence or
as requested by RN Supervisor. Accompany physicians on rounds as
needed. Review resident care plans and Medication Administration
Records (MARs) with supervisor and physician. Document all patient
information in resident charts and ensure completeness. Assemble
and submit resident charts to Medical Records Department upon
discharge. Take and record vital signs as required. Notify
physician and Director of Nursing of any unauthorized resident
discharges. Instruct residents in self-help and self-care
activities. Assist residents in preparing for appointments and
arrange transportation as needed. Perform tests and other
diagnostic procedures as ordered by attending physician. Assist
with restorative and rehabilitation programs for residents.
Administer medications and treatments as prescribed by attending
physician. Maintain inventory of resident medications; order as
needed. Review physician orders to ensure accuracy of medications
listed on MARs. Assist with feeding residents when necessary;
monitor diets and record intake and output as ordered; report needs
to nursing or dietary supervisors. Report any discrepancies in
orders, diets, etc., to nursing supervisor. Responsible for
post-mortem care of residents. Participate in ongoing in-service
and orientation programs. Perform minor housekeeping duties within
nursing unit. Maintain calm demeanor during emergencies. Ensure
noise levels in nursing center are appropriate at all times. Report
needed repairs to maintenance department promptly. Perform daily
cleaning and maintenance of medication carts; ensure stocked with
necessary supplies for upcoming shifts. Perform prescribed
treatments on residents in assigned unit; document completion of
treatments daily and assess skin integrity weekly. Check residents
frequently as indicated. Prohibit smoking in bed for residents;
ensure attendants or visitors supervise smoking in designated
areas. Enforce smoking regulations among personnel, visitors,
patients, and the public; report violations to nursing supervisor.
Ensure nurse call system is accessible to bed-bound residents at
all times. Report any instances of resident abuse immediately. Wash
hands after caring for each resident. Follow facility infection
control procedures, especially during isolation techniques. Respect
and uphold resident rights to fair treatment, self-determination,
privacy, property, civil rights, and the right to file complaints.
Foster a warm, positive environment while maintaining resident
confidentiality. Access only necessary confidential medical
information for residents under direct care. Qualifications Must
hold a current and unrestricted license as a Licensed Practical
Nurse in the state of North Carolina. Good general health and
emotional stability required. Must possess patience, tact, a
cheerful disposition, and enthusiasm for working with residents at
varying maturity levels.
