Job Description Job Description We are seeking an experienced and compassionate professional with expertise in aging services to support older adults and their families. This position plays a critical role in connecting clients to essential community resources and ensuring high-quality service delivery. Key Responsibilities: Conduct comprehensive, holistic assessments of clients Provide information, referrals, and service navigation support Manage centralized intake and client counseling Maintain accurate client records and documentation Update and manage community resource directories Plan and coordinate service-related presentations and outreach activities Qualifications: Experience in Social Work or a related field preferred 2–3 years of experience providing social services to older adults and/or families Strong knowledge of supportive services and local community resources Demonstrated advocacy, organizational, and problem-solving skills Ability to manage multiple responsibilities while achieving positive client outcomes We are looking for someone who understands that aging services is not paperwork—it’s people work.
